US Club Registration

Please make sure you have completed the required forms.

The required forms are the Medical Release Waiver and the Parent/Player Agreement form. You can print these out through the links and bring them already signed or just fill one out on the evening. We will also need you to bring a copy of your childs birth certificate. Player Cards are required so will be taking a picture of each child for the card.
